This machine-readable file is not necessarily intended for direct patient interpretation. This information has been compiled from our billing, claims, and insurance payment files.

Price Transparency Information
The US Department of Health and Human Services, US Department of Labor, and US Department of Treasury, effective 7/1/22, non-grandfathered health plans (which include employers who sponsor employee benefit plans) and health insurance issuers must publicly post pricing data known as the "machine readable files" or (MRF's).
